Please note:

• For sole proprietors and partnerships (if married in community of property), provide the spouse’s ID and their name and signature on the credit application and suretyship.
• Full names and ID copies of all directors, members, partners, and owners are needed.
• Include proof of physical address (e.g., utility bill) in the business’s name; PO Box addresses are not accepted.
• Indicate insurance preference (Kenings or your provider) and provide a copy of the policy if using your own.
• The form must be completed and signed by a proxy. Supply trade references with full details such as company name, account number, branch, contact info, credit terms, and purchase details.
